Classroom Volunteer

When a parent, community member, or professional volunteers time in a classroom of Head Start (HS) or Early Head Start (EHS). Note there are two different forms: One for parents and community members and one specifically for professionals.

Meeting Sign-In Sheet

For any meetings, such as parent meetings, transition meetings, or socialization activities. Use one form per classroom, with each adult printing and signing their name. Note parent meetings are to have an allowable time of only 0.25 hours per meeting unless otherwise noted on the form with reasons why the allowable time was longer. Note time spent in socialization activities is not allowable but travel time and mileage do count.

Reduced Rate Professional Services

When professional in-kind is donated as a percentage or a lump sum, or when an hourly rate is deducted off an invoice. For example, a doctor may discount the invoice by 40% or discount $5 off per hour, this deduction counts as in-kind. If the professional is volunteering merely time, use the normal Professional Classroom Volunteer form.

Non-Classroom Volunteer

When a parent, community member, or professional volunteers time somewhere other than in an LCECP classroom (e.g. reception, administrative data entry, washing dress-up clothes at home, fluffing playground bark). Note there are two different forms: One for parents and community members and one specifically for professionals.

Parent/Staff Home Visit

When staff conducts a visit in the home to discuss a child’s progress, goals, etc.
with the family.
Only space value is allowable.

Parent/Staff Center Visit

Similar to a parent meeting, but only for the child’s family members. Meeting time is not allowable, but travel time and mileage do count.

Letter for Reduced Rate Professional Services

When a letter is provided stating the full rate and discount rate for services. The difference counts as in-kind. Attach the letter and invoice behind this form.

Home
Activity Sheets

When daily activities are performed by the child and checked off weekly by adults in accordance with the teacher’s goals set for children. Each checkmark equals ¼ hour. For each day of the week, only four checkmarks (1 hour) can count, although of course more activity with the child is encouraged.

Group Volunteer Sign-In Sheet

When an agency or group of professionals or community members volunteers together (such as Idaho Department of Corrections or LCSC Nursing Students). Note there are separate forms for community members and for professionals. If the professionals have different wage rates, use separate forms. Do not use this form for parents.

Donated Items
Supplies

When an item is donated that is allowable under the non-federal share guidelines, i.e., for use in the classroom, not for an individual or family. If a receipt is available, attach it to this form.

Room Rent Donation

When use of a room is donated for LCECP purposes.
